Referral types
- Self- referrals – you can refer yourself to us
- GP referrals – an Enhanced Primary Care Plan referral (ECP) through your General Practitioner, may enable you to claim a Medicare rebate.
- Endocrine referral – referral by your endocrinologist

- Telephone and telehealth
- After hours and weekends
- Home visits – can be organized upon request
What to Bring for appointments
Bringing the list of your current medications, blood glucose monitoring device and diabetes management diary if one is being maintained and copies of most recent blood tests ( HbA1c/ Lipids/ general biochemistry/ microalbuminuria) will be very useful for the comprehensive CDE assessment. (your GP or practice nurse can help with this)
